Assisted Living Centers Background Information

Assisted Living Centers, Inc. was created to own and operate assisted living homes for the elderly. The mission is to provide the opportunity for our residents to experience the highest quality of life possible while maintaining a safe and nurturing environment. The focus is to provide an alternative to institutional living. By using large homes, in residential neighborhoods, and modifying them to comply with applicable state and local regulations we are able to furnish and decorate each property in a way that provides a comfortable, home-like atmosphere along with a well trained and caring staff to provide for the needs of each resident.

Starting in January of 1999, Assisted Living Centers, Inc. has grown to encompass six assisted living homes providing 90 resident beds for occupancy. At one of our gracious homes located in Baltimore County, Maryland, you will experience all of the comforts of home, but none of the work or worry. Many of our homes are turn-of-the-century mansions designed to provide you with the highest quality of care and socialization.

Room Reservation Agreement
Health Care Practitioner Physical Assessment - To be completed prior to physical assessment of resident.
Resident/Provider Agreement - Must be completed before admissions.
Resident Application for Admission - Must be completed before admissions.
Do Not Resuscitate (DNR) - Needs to be completed by a physician or waiver signed by a family member.
HIPPA LAW - If resident is in a medical facility/Assisted Living/Rehab/hospital/nursing home, the HIPPA form needs to be completed prior to assessment to enable us to review medical charts.
